Proof of international medical insurance while in Spain, issued by a Spanish insurance agency.

Police Criminal Record clearance certificate including fingerprint verification, issued less than 3 months from the application date. In the event that you are also applying for your spouse, your spouse also requires a Police Criminal Record clearance certificate. Additionally, police records from any other countries where you have lived during the past 5 years are required. The Police Criminal Record clearance certificates must be Apostille legalised.

Medical Certificate, in the form of a doctor’s statement issued less than 3 months from the application date on the letterhead of the doctor or medical clinic, signed by an M.D. The statement should confirm that ‘the patient has been examined and found free of any contagious diseases according to the International Health Regulation 2005’.

Dependants – in the event that the application includes family dependants, a Marriage Certificate (legalised by Apostille) is needed for the spouse, and a Birth Certificate (legalised by Apostille) is needed for each dependant under 18 years of age.
